★ ★ ★ ★ ★ There will be only two staffs to handle customer service and only they will do all work. They are Cooperative very well and they work better in discipline. All you need to do you should bring all papers related documents of Employment Provident Fund to like with signature Pensioner's bank manager or gadget officer.
1. Aadhar card photocopy of both expired person and yourself.
2. Bank Account Passbook photocopy of both person.
3. Death certificate of death person (due to covid 19)
4. Documents of covid 19 proof or hospital report.
They will provide a form which Pensioner's have to fill with their information like
1 Name of member who has expired by covid 19.
2 Their EPF Account Number.
3 Name of their pensioner (who will get their pension after patient's death.)
4 Their Nationality either Indian
5 Pensioner's height
6 Pensioner's any two identity marks like any face cut mark, any wound marks or eyes colour ??
7 After that fill with 3 signatures at three blank lines, or four fingers print one at each four box if pensioners is illiterate.
8 At last Pensioner's signature or any fingerprint is required at last right corner with bank manager's signature.

★ ★ ★ ★ ★ Required few documents with bank manager's signature at each i.e.
1.Bank Account Passbook photocopy of both expired person (by covid19) and Pensioner's too but both should have Account at same Bank from either SBI or PNB.
2.EPF Account Number of expired person
3. Aadhar card photocopy of both person. All these photocopy are enough.
They will provide a form that have to be filled by Pensioner also have photocopy of this also signature of bank manager. So have one photo with yourself and submit the original copy with all above 3 documents attached.

★ ★ ★ ★ ★ I visited the EPF Office situated in Nandgram with my grandmother to fill her pension form. The Office is on the ground floor of a commercial building and there are new construction is going on. The area is a market place and there is no space to keep your car park. So, i had to leave my car quite far from there. The Road standard is bad and no one can go there in rainy days.
