Hospital has to submit online and they will generate a temporary Id.submit form with Xerox of following
1.take form 4 from hospital
2.address proof of deceased
3.id proof of deceased
4.Id proof of submitters
Certificate will come at home address in 7-10 days

★ ★ ★ ★ ★ Official people give proper answers and behave nicely but processing time is very much. I wanted my birth certificate in English language and they took almost 1 and half to 2 hours for just print and authorize one single copy. To get copy at the same day price is 50 + 20 searching charge. Total is 70 rs. And if it's not urgent then 30 + 20 searching charge. Total 50 rs. You can collect it after 10 days.
