★ ★ ★ ★ ★ The forms are available at the counter or can be purchased at the nearby paan shop. Forms are available for free at the counter. Submit at the counter after filling appropriately. Ensure records from hospital have been submitted to here. It takes 8-10 days for verification and the certificate can be collected at the counter by anyone with the receipt. Agents or similar are available outside gate to help if you can spend some extra penny, thanks to our corrupt system
