★ ★ ★ ★ ★ हर प्राथमिक स्वास्थ्य केन्द्र के ३०००० आबादी के लिये ४ से ५ उप स्वास्थ्य केन्द्र होते हैं। स्वास्थ्य उपकेंद्र ५००० लोगों के इलाज के लिए बने होते हैं। हर प्राथमिक स्वास्थ्य केन्द्र में एक या दो डॉक्टर, दो नर्स या दाइयॉं, ५ – ६ ऑक्सिलरी नर्सें (उप केन्द्रों में), क्लर्क, वार्ड सहायक और फील्ड सुपरवाइज़र होते हैं। एक वाहन भी वहॉं होता है। एक प्राथमिक केन्द्र के काम ये हैं –
बहिरंग रोगी मरीज़ों का इलाज।
खास तरह के क्लिनिक में गर्भवती और दूध पिलाने वाली माताओं की देखभाल।
बच्चों के स्वास्थ्य सेवा जिसमें टीकाकरण भी शामिल है।
राष्ट्रीय स्वास्थ्य कार्यक्रमों जैसे तपदिक, कोढ़, मलेरिया, अंधेपन, फाइलेरिया या दस्त आदि के साथ परिवार कल्याण कार्यक्रम और टीकारकरण कार्यक्रम को लागू करना।
समग्र बाल विकास कार्यक्रम में पूरक भूमिका।
स्वास्थ्य कानूनी सेवा जैसे असामान्य मौत, लड़ाई में लगी चोटों, बलात्कार आदि की जांच करना, प्रदूषण नियंत्रण के उपाय और खाने की जगहों का निरीक्षण।
पानी के शुद्धीकरण और स्वच्छता के मुद्दों पर काम में मदद।
समुदाय में स्वास्थ्य शिक्षा फैलाना।
पराचिकित्साकर्मी स्वास्थ्य कार्यकर्ताओं, सामुदायिक स्वास्थ्यकर्मियों और दाइयों को प्रशिक्षण देना। उप -केन्द्रों में आमतौर पर एक पुरुष स्वास्थ्य कार्यकर्ता और २ सहायक नर्स होती है। उनकी ज़िम्मेदारी होती है कि वे –

स्वास्थ्य शिक्षा का आयोजन करें।
आम बीमारियों के लिए बुनियादि स्वास्थ्य सेवाएं उपलब्ध करवाएं।
 Every primary health center has 4 to 5 sub-health centers for 30000 population. Health subcodes are made for the treatment of 5000 people. Every primary health center consists of one or two doctors, two nurses or midwives, 5 - 6 auxiliary nurses (in sub centers), clerks, ward assistants and field supervisors. A vehicle is also there. The functions of a primary center are:
Treatment of Outpatient Patients
Caring for pregnant and feeding mothers in a special kind of clinic.
Children's healthcare, which includes immunization.
Applying the National Welfare Program and Family Welfare Program and Immunization Programs such as Hurricanes, Leprosy, Malaria, Blindness, Filaria or Diarrhea etc.
Supplementary role in the overall child development program.
Examine health legal services such as abnormal death, in-fighting injuries, rape etc., pollution control measures and observation of food places.
Help in working on water purification and sanitation issues.
Facilitate health education in the community.
