Don't let the patient speak.
When asked about the cause and treatment of the condition asked me to look on Google.
Treats like as if he works in some trust and patients are coming for free without paying fees.
Should work on his attitude.

This doctor was one of the best one i know but it's been so long now the thing has been changed he and his son are just a money making machine.
