★ ★ ★ ★ ★ Today ,my booster dose was schduled between 11.00 to 12. 00 in the afternoon. I reached the hospital at around 11.00 . I was just asked to sit. Till 11.30 nobody informed me to fill form .When one person was filling the form I came to know the practice . The person at the reception was not writting the names to ensure first in first out system , neither asking to fill up the form .
Fortunately the nurse injecting was very skilled .
