Board ST bus from Thane railway station (₹30) or Kalyan railway station and reach Kalyan Naka (OR board TMT from Chendani Koliwada and reach Narpoli). Board Autorickshaw to Dhamankar Naka (₹10). Then walk for 15 minutes for your good health to reach the center. Get down at Bhiwandi bypass then share rik till Dhamankar Naka and 10-15 minutes walk from the Dhamankar flyover.

★ ★ ★ ★ ★ Worst vaccination center ever . No washroom facilities nothing, the people working here the administrative staff does not wear mask at all ! The room where people are vaccinated has stagnant rainwater inside it which attracts mosquitoes, I know the effort here is to give the vaccines for free but atleast sanitize the area and make it clean so that people don't catch other infections ?.
In conclusion it's better to pay for vaccines than come to places like these for vaccination !
