★ ★ ★ ★ ★ It is. Respectfully submitted that at our place at Sola Sai Dham, your unit is established for getting services by people at large. I being the person having corona positive through R T PCR report through blood collection and report made by your lab through home attendant on 2/4/21 and thereafter on 4/4/21 or 5/4/21, therefore appreciated the services, however with pain I am compelled to say that though massages are sent through Gautambhai , a trustee of Sai Dham, about sending of report’s physical copy with receipt of payments, complete wilful negligence and or wilful avoidance, which is required to be taken for consideration. Just for your information I don’t want to introduce my self, but only to request you to instruct the office staff to send physical copy of report on your later head showing authentication with receipt of payment, as it is mandatory to submit it to air port authority. For me as I was to fly to day to USA, unable, however I have to give the said documents to concerned air line authorities It may be treated as urgent. No contact number , no email account particulars on your report..................Balkrishna Patel Aged 67
