A income certificate for last year from Tahsil office,
And yellow/saffron ration card.
Just ask for the help at accounts and charity department.
If hospital staff don't listen then you can contact charity commissioners office.

Consultation fee is 600Rs for first visit.
Appears spacious, hygenic and costly hospital to me...
Parking Charges are 10 and 20 Rs inside for 2/4 wheelers.. which make no sense to earn money.
